Therapeutic care model
Assure Healthcare Group’s service user-centered care model employs a ‘we’ approach that offers Service Users the following:
- Social Care Team
- Therapeutic supervision;
Staff learning & development
- Care Team / Service user family
- Registered Manager
From Transition aged adults to end of life, we employ the same therapeutic care model with our ‘we’ approach to the care services we provide. The ‘we’ is a consistent approach with the therapeutic underpinning of a psychological therapist who provides supervision and training to ensure the care teams work with our young people and adults in a therapeutic way.
The ‘we’ provides a consistent approach that allows for the service user to feel contained and safe, as it helps to ensure teams are communicating and not working in a chaotic, or random manner.
Our service users are always at the centre of the ‘we’ and their families and our care teams (AHG + social care) are the ‘we’. This ensures that we are all communicating the same message, thus continuity is ensured and provides that the service user feels safe.
At Assure Healthcare we work every day to create a care family where all of our service users feel valued and respected, so they have the confidence to grow and have aspirations that allow them to lead the lives that they deserve.
The ‘Home for Life’ philosophy that we promote, requires that everyone knows where their journey is taking them. We always endeavour to never end placements, and within our homes, to find the right placement for that time in the moment of their lives.
Learning disability and mental health disorder does not change due to a number representing age; it comes along the road for the journey at Assure Healthcare Group. We want to limit public expense and the inevitable consequences of placement changes.
